Application for Admissions
If you are interested in attending Manchester Community College as a matriculated student, complete the above application or apply online.
Affidavit of Support
Form to be completed by student seeking admission as an international student on a F-1 Student Visa, and by the student's financial sponsor.
Eligibility Confirmation for Transfer by an F-1 Student
If you are an international student transferring from another SEVIS-approved institution, you must complete this form and submit it with previous I-20s and a copy of your I-94 card.
Health Record / Questionnaire
Every student accepted into a program of study at Manchester Community College must complete a health form which details their medical history.
International Address Submission
If you are an international student seeking F-1 student status, you must complete this form as part of your admissions process.

Accuplacer Testing Form
As a new student, you are required to take the Accuplacer placement test. Complete the form in order to take the assessment.
Health Insurance for 2009-2010
A special accident and illness insurance policy is available to all students enrolled at MCC. You are encouraged to either provide your own coverage or purchase the insurance provided by CCSNH.
Parking Permit Registration
All students are required to have a parking permit on their vehicle if parking on campus. Submit this form to the Office of Campus Safety or the Receptionist.
TRIO Application
Low income students, first generation college students, and students with disabilities can apply to TRIO to receive additional personal and academic support to help them achieve their academic goals.

Change of Major / Dual Degree
Complete this form if you would like to change your major, or complete more than one degree at the same time.
Drop / Add Form
Within the withdrawal period, students are allowed to add or drop a course, with approval from the instructor.
Prerequisite Waiver Form
Prerequisites for courses can be waived if the instructor believes the student is capable of succeeding in the class without the necessary course requirement(s). Complete the form and return to the Registrar's Office at the time of registration.
Refugee Scholarship Packet
This scholarship was established specifically for students who came to the United States as refugees. Four $567.00 scholarships will be awarded to be used for tuition for the January, 2010 semester.
Registration Form
For current students who would like to enroll in courses for any term. Students can also enroll online through the Student Information System (SIS) after completing their first semester at MCC.
Transcript Request
To have your academic record sent to another school, please fill out the above form and submit to the Registrar's Office.
Tutor Request Form
Even though you can just drop-in for tutoring in the new Math & Writing Lab, you still have to complete a form when you come in or beforehand to receive help.

Petition to Graduate
Students who have completed their academic requirements and wish to receive their degree, must complete this form in order to finalize their graduation.
Senior / Alumni Permanent Address Form
Help the college keep in touch with you after you graduate by completing this form. If there are any important notifications or documents that need to be sent, we need to have an up-to-date address.
Transcript Request
To have your academic record sent to another school, please fill out the above form and submit to the Registrar's Office.

Facility Use Request Form
If you are hosting an event, the facilities and classrooms at MCC are available for rental through the Workforce Development Center or Registrar's Office.
